Sanitary Sewage Overflow Reporting

All publicly owned treatment works and privately owned domestic treatment works that own and operate a sewage collection system, including satellite sewage collection systems, must report to the Department all occurrences of sanitary sewer overflows or treatment facility overflows using the Sanitary Sewage Overflow Notification Summary Report (form 3400-184). This report is available on Switchboard and must be submitted electronically through this portal. One form per overflow location should be submitted.

Please complete the form according to the instructions within 5 business days of the event, as is required per s. 210.21(4)(b), Wis. Adm. Code and WPDES permit standard conditions.
