REPORT ID REQUEST FORM

Use this form to request a Report ID to allow you to submit an NR700 Activity progress report. You must enter a valid BRRTS number without dashes for an Activity requiring semi-annual reporting for the current round. The Report ID you will receive uniquely identifies the Activity, and the reporting period, and verifies the person using the ID is authorized to submit the report. You will receive an email containing a URL for you to access the report form.
