The Operator Certification program added an additional quarterly exam date on February 7th, 2018. This opportunity is in addition to the main exam dates in May and November. It is open to all operators and registration will be on a first come, first served basis. Applications will be accepted until the site is full.

To register, operators must send an exam application with payment to the address on the application. The deadline to apply is January 10th, 2018. All registered applicants will receive a confirmation letter in the mail.

Note: The Social Security Number Collection Request Form (page 2 of the exam application) only needs to be completed if it is the first time an applicant is taking an operator certification exam.

Veterans fee waiver program
Veterans may apply for a one-time fee waiver to be used towards exam fees. Refer to the veterans fee waiver program information [PDF] for more information.

Forms

Training Report Form

Operator Training Report (Form 4400-190) [PDF]

Use this form for any relevant training that was not pre-approved by the DNR (such as in-house safety training) or for some online training. Training approved with this form will be added to an operator's Training History Report.

  1. Type directly within the PDF form.
  2. Save a copy for yourself.
  3. Send via email to DNROpCert@Wisconsin.gov. Please do not email a scanned version of the form.
  4. The subject line of the email should read "[Training Course Title] - Operator Training Report Form".
  5. Attach the proof of attendance to the email (i.e. certificate of completion or a copy of the sign-in sheet).

Solutions to PDF issues

Some browsers use a PDF viewer that does not support dynamic PDF forms. In particular, Chrome and Safari are generally defaulted to their own PDF viewers rather than Adobe Reader. Using Internet Explorer will usually resolve the issue; otherwise two solution options are:

  1. Reconfigure your browser [exit DNR] to work with the Adobe Reader or Acrobat plug-in, or
  2. Save a copy of the form to your computer and then open it using Adobe Reader [exit DNR].

Experience and Reciprocity

Reciprocity

If you have a current certification in another state and took one or more exams to gain that certification, then you may apply for reciprocity. If you are applying for both waterworks and wastewater certification, complete a separate application for each. A complete reciprocity application includes the following:

  1. Wastewater Operator Reciprocity Application [PDF] or the Drinking Water Operator Reciprocity Application [PDF]
  2. Social Security Number Request Form [PDF]
  3. Cover letter or resume describing your overall experience
  4. Copy of your current certificate
  5. Either a Wastewater Operator Certification Experience Form [PDF] or Waterworks Operator Certification Experience Form [PDF]. Note: Complete a separate experience form for each facility that you have worked at. For previous employers, you may enter your former supervisor's name and phone number on the form (a former supervisor's signature is not required).
  6. If you would like to apply for advanced level certification in wastewater, then also complete the Advanced Wastewater Treatment Plant Operator Certification Application [PDF]. 4 years of hands-on experience is a minimum requirement for advanced level certification. Mail this form in with your reciprocity application.
  7. Check or money order for $100

After receipt of your application, please allow 1-3 months for the verification and review process.
