The following documents are intended to assist individuals who complete the monitoring forms for facilities issued a specific WPDES permit. These documents have previously been included in mailings of monitoring forms to the persons responsible for completing the forms. They are provided here in PDF format as an additional resource.

Instructions for:

  • Groundwater monitoring forms
  • Monthly discharge monitoring report forms (long forms): paper or electronic (web)
  • Quarterly/semi-annual/annual discharge monitoring report forms(short forms)
  • Sludge/waste characteristics report form(form 3400-49)
