CMAR Certification Forms


There is one final step to complete after you have submitted your eCMAR form. You need to mail the Department a separate CMAR Certification Form provided by the Department. The CMAR Certification Form is separate from the eCMAR submittal process because the Department is required to have a signed paper copy of the CMAR Certification Form until such time as electronic signatures are legally acceptable.

When your authorized submitter clicks on the “Submit” button on the eCMAR form, they will be prompted to print out a page entitled, “Electronic Compliance Maintenance Report (CMAR) Certification.” Print this page out, fill in the required information, have your authorized representative sign it, and mail it to the Department as soon as possible after the submittal of your eCMAR form. The authorized representative is usually a person in a managerial position within the municipality or private entity such as clerk, director of public works, city engineer or coordinator, or elected official who is designated by the governing board to sign the CMAR certification form.

The CMAR Certification Form shall be signed by the authorized representative of the treatment works owner verifying that submittal of the eCMAR form is complete, accurate, and represents information from the owner’s treatment works.
