Hazardous Waste -- Waste Received From Off-site (WR) Form

The Waste Received from Off-site (WR) Form is required for annual hazardous waste reporting in odd-numbered report years. The completion of the WR Form is required for any sites that received hazardous waste from off-site during the report year. The hazardous waste Web reporting system determines which sites need to submit the WR Form.

Purpose

The WR Form collects information about the quantities and characteristics of each hazardous waste received from an off-site source and the method(s) used to manage them.

WR Form Completion Requirements

Generally, the WR Form is completed by licensed treatment, storage and disposal facilities (TSDs) that receive hazardous waste from off-site during the reporting year or by publicly owned wastewater treatment works (POTWs) that accept hazardous waste.

WR Form Screen Instructions

Please note the following regarding the WR Form screen:

  • In the Web reporting system, each type of hazardous waste and off-site source reported in the last “long” report year will be listed in the upper part of the screen.
  • Edit or delete waste streams in the list to reflect the waste received from the off-site sources during the current report year.
  • If no waste records are displayed for your facility (only an empty record), add each type of waste from an off-site source you need to report on. Start by editing the empty record in the upper part of the screen. Then click the Save button and your first waste record will be added.
  • Add more waste streams by completing the area below the list with waste code, off-site EPA ID form code and description; then click the Add button to the left. In order to successfully add a waste record, each set of waste code, off-site EPA ID and form code must be unique.
  • When the list is complete, click the radio button to the left of highlighted waste.
  • Complete the lower section of the screen for this waste giving details about the amount of waste received and how it was managed.
  • Add comments about this waste stream in the Comments section.
  • Click the Save WR Information button.
  • A check mark will appear in the Complete column for the highlighted waste stream when all information is complete.

Alternative Submittal of WR Form Information

For sites needing to report a large number (20+) of the WR Form, there is an alternative submittal method outside the Web reporting system. Send an email with your site information (facility name, EPA ID and Facility ID number), requesting the WR Form specifications. File specifications and instructions will be sent by email.
