Hazardous Waste Identification/Certification (IC) Form

The hazardous waste Web reporting system determines if a site needs to complete the IC Form, based on your answers on the reporting requirements screen.

Purpose

The purpose of the IC Form is to continue identifying the hazardous waste related facility that you are reporting for and the activities associated with that facility. The information collected here is the same as on the EPA Form 8700-12 Site Identification, which was used when an EPA ID number was initially obtained for the site.

IC Form Completion Requirements

The Hazardous Waste Annual Report Identification/Certification (IC) Form is required if the facility needs to report information about hazardous waste amounts. This requirement applies if the site is a large quantity generator, a small quantity generator or a treatment/storage/disposal facility. The Web Reporting System will supply the Exemption Form to the user if the facility is not required to report information about waste amounts (sites that did not treat hazardous waste on-site during the reporting period and are either a very small quantity generator or a non-generator).
