Hazardous Waste Identification (IC) Form

IC Form

Required for sites that were LQGs, SQGs, TSDs, POTWs or Permanent Collection Facilities during the report year.

Site activity information collected for both the report year and current year on the "reporting requirements" screen for all sites.

The hazardous waste Web reporting system collects site activity information on the reporting requirements screen and on the IC Form. The system will determine if your site needs to complete the IC Form, based on your answers on the reporting requirements screen.

Purpose

The purpose of the IC Form is to collect additional identifying information about the hazardous waste related facility that you are reporting for and the activities associated with that facility. The information collected here is for the current year and is similar to the EPA Form 8700-12 Site Identification, which was used when an EPA ID number was initially obtained for the site.

IC Form Completion Requirements

All sites are required to provide the minimal site activity information collected on the reporting requirements screen, for both the report year and the current year. The Identification (IC) Form of Hazardous Waste Annual Report is required for sites that were in any of these activity categories during the report year:

  • Large Quantity Generator (LQG)
  • Small Quantity Generator (SQG)
  • Treatment, Storage or Disposal (TSD Facility
  • Publicly owned (wastewater) treatment works (POTW) that accepted hazardous waste (via truck, rail or dedicated pipe) for treatment and complied with s. NR 670.001(3)(b)9.
  • Permanent Household and Very Small Quantity Generator Hazardous Waste Collection Facility that ships hazardous waste off-site to a licensed or permitted hazardous waste treatment, storage or disposal facility.
The Web Reporting System will supply the Exemption Form to the user if the facility is not required to report information about waste amounts (sites that did not treat, store or dispose of hazardous waste on-site during the reporting period and are either a very small quantity generator or a non-generator).
