Leaking Underground Storage Tank (LUST) Reimbursement Claims
American Recovery and Reinvestment Act of 2009 (ARRA)

Wisconsin received $6.381 million in Leaking Underground Storage Tank (LUST) Recovery Act dollars. The LUST funds will be used for assessment and cleanup where there are no viable responsible parties (RPs), where remediation is fairly ready to proceed, and where the site is part of a pending or potential redevelopment action.

How do I submit a claim?

The LUST Reimbursement Claim Worksheet (Form 4400-256) [PDF] is a PDF fillable form created specifically for recipients of the Leaking Underground Storage Tank funds from the American Recovery and Reinvestment Act (ARRA).

This form should be used when submitting reimbursement requests. Please fill out this form, attach all invoices (and documentation) and mail two (2) completed copies to:
