Notification of Demolition and/or Renovation and Application for Permit Exemption

If you plan to demolish or renovate a building that may contain or is known to contain asbestos, you must notify the DNR at least 10 working days before starting the work, except in emergencies, using the form "Notification of Demolition and/or Renovation and Application for Permit Exemption." You should fill in the form, print it, and mail it, along with applicable fees, to the appropriate DNR office.

• Filing a Complete Notification, Form 4500-113 (Power Point slide show, revised 8/2005)
