Application and Permit for Collection and Research in Wisconsin State Natural Areas (9400-280)
This application allows an individual to apply for a permit to collect and research in the State Natural Areas of Wisconsin. Please submit your application within 10 business days of your collection/research date to allow for review. View Form [PDF 212KB]

Endangered and Threatened Species Form


Endangered and Threatened Species Permit Application (1700-001)
The Wisconsin Endangered Species law allows the Department to authorize the taking, exportation, transportation or possession of listed plant or animal species via an Endangered and Threatened Species Permit if the activity is for zoological, educational, or scientific purposes, or for propagation in captivity for preservation purposes. Endangered Resources Review Request (1700-047)
Use this form to request an Endangered Resources (ER) Review of a proposed project. You may fill this form out on your computer, print, and mail or fax it to the ER Review Program. However, you can not save the form, as any filled-in fields will be lost. View Form [PDF 623KB]

Voluntary Expedited Endangered Resources Review Request (1700-047E)
Use this form to request a Voluntary Expedited Endangered Resources (VEER) Review of a proposed project. A VEER Review is essentially a regular ER Review completed in a guaranteed, short timeframe for a higher cost. The guaranteed turnaround time for VEER Reviews is 7 working days. You may fill this form out on your computer, print, and mail it to the ER Review Program. However, you can not save the form, as any filled-in fields will be lost.

Wisconsin Colonial Waterbird Online Survey (1700-058)
This form was designed to streamline the data submission process for professional wildlife biologists and experienced volunteer cooperators who have conducted or are conducting colonial waterbird surveys during the course of spring and summer outdoor activities. View Form

The following field report forms are for use by field observers when recording observations of rare plants or animals. The forms provide standardized information that will help us to enter the data and to interpret the observation for that sighting (and in the larger context of Wisconsin). The form should be filled out as completely as possible and sent to the address listed at the bottom of each form. The Natural Heritage Program cannot accept data derived from trespass. Gain landowner's permission before entering private property.

Natural Heritage Inventory (NHI) Rare Animal Field Report (1700-048)
